Your First Visit
Please arrive 10 to 15 minutes early so that you have enough time to fill out a New Patient form. To save some time, you can also download and print the New Patient Form and fill it out in advance.
Initial Examination
Your dentist will start with a review of your medical and dental history, then complete a comprehensive dental exam. The exam will consist of an extensive examination of your gums, teeth, bite and jaw, an oral cancer screening and any X-rays that might be required.
Treatment Plan
Taking your history and exam into account, your dentist will develop a long-term dental treatment plan customized to your needs. Additionally, they will take the time to answer questions you may have about your dental health and treatment options.
Insurance & Payment Information
At Uptown Dentistry, we'll help guide you through our payment, insurance, and financing options so you are able to focus on achieving your dental goals.
Payment Methods
The treatment cost must be paid in full at the time of your appointment. We accept payment by Visa, MasterCard, and e-transfers.
Direct Billing
We will submit the amount owing to your insurance company on your behalf. You will be responsible for any remaining balance your plan does not cover. Our administrative team would be glad to assist you in better understanding your insurance plan, so don't hesitate to ask questions.
